Shoota for the Doc? Nah, not required. But more then a knife-tier only Melee weapon would be nice.

Something like a Chain-Choppa or Doc-Saw would be sweet

Or a Painclaw. In my head just like a powerfist but without the AV Capabilities. Maybe too overpowered but would make the painboy unique

My favorite class is meant to be booring, don't try.
