Not necessarily achievement based, but for being an outstanding member in the space marine community. Kinda like the Jedi in star wars galaxies, but easier to achieve. The player should reach a certain height in team based stats (such as holding points, revives, squad based actions) and have no history of hacking, cheating, or trash-talking to ensure that the player being awarded with Terminator Honors knows what he/she is doing and is a team player who will use the armor to help their faction, and not their stats.
Naw, it should be on a character by character basis. It shouldn't be something that the entire team gets for a duration.
I belive that the Terminators will be acquired as the same as others field assets (Tank, Rhino, Bike's... ), even from the same requisition pool, at a higher level, being expensive, and with a longer cooldown. Making the player choose between the strategic assets.
Librarians are not as common to a Chapter as a Terminator armor suit. It would be extremely strange to have more Librarians than terminators. Also with them having to fit a Hero class, i disagree. Make them a support caster type thing with some decent close combat ability and that is fine. they are not at the same level in combat as captains or chapter masters so as heroes, i dont think its necessary.
I have another question for terminators all together. Will we see chaos Terminators? They're just as iconic, in my opinion. Perhaps they could be more melee centric as opposed to the loyalist ones? This can be explained with that the loyalists have newer gear and what not. Chaos could get small mutations?
Depends on the Chapter. Some Chapters can field nearly a company of Librarians and have nearly 0 Terminator Armor.
What chapter exactly? Grey Knights are the only chapter i know that have a plethora of psykers and thats by design. which one?

Bluhd Rehvens for one being the most well known. Though they can field Terminator Armor (It's debatable seeing as the ending of Retribution left a great deal of the 1st Company wiped out. The Crimson Fists also had a larger than average stock of Librarians prior to their whole fiasco with Rynns World where again a good portion of their First Company was wiped out along with their entire Fortress Monastery. There are a few more which have next to no Terminator armor still in service and have an active Librarius. I'll have a look and get back to you, can't remember off the top of my head.
