If you are killing for pleasure you are doing so because you get pleasure from killing. However, you can get pleasure from killing while not killing for pleasure, I.E. you are killing these particular people because someone told/paid you to. Do you enjoy swatting flies and stepping on cock-roaches?
Agreed. It is the one sticking point I have with that story. It's a bit to ambiguous for my liking (which was probably his intent anyway!).
In other words, never specified. Much ado about nothin'. Aside from the basic premise of the character being immortal she's the faction leader of the game. She'll be back before you can say Rhana Dandra.
