the sacrifice move (at least in my head) goes a bit like this: you're in a tyranid instance, or perhaps in pvpbattle aswell (we dont know too much about pvpbattles so it remains to be seen), your character is on its last 5%, and you decide to make the move. you cant make the move when you get killed, its not martyrdom like CoD. It's sorta an achievement move. It's also true to the spacemarine's nature for example. I don't see Orks doing it, but maybe each race can get it's own unique achievement or ability move. Besides, I notice several posters here taking each suggestion literally to the letter. Stop doing that. We don't know about gamemechanics and therefor all suggestions are more trains of thouht rather than concrete concepts.
so just like in space marine where they have final vengeance (I think it's called that) which you do get the kill/ assist for. but I think that the game will have plenty to encourage support for your faction's war effort without needing a "suicide move"

I like the idea with seeing the description but having kamikazes is a terrible game design decision People are gonna exploit that
I like the idea of limiting the number of "badges" that someone can display. Eventually people will have many of the same just through continued play so in this way each person can have a way to advertise their speciality to their squad.
